WAIVER: I, the volunteer participant (and my parent or guardian if I am younger than 18 years of age), intending to be legally bound, do hereby and as attested through the completion of my Move4Mona Registration Form and the payment of $45 registration fee, acknowledge that I have read and agree to assume all risks which are foreseeable and involved with or may arise out of my voluntary participation in Move4Mona 5K Walk/Run Challenge (or my child’s voluntary participation in Move4Mona 5K Walk/Run Challenge). 

Further, I forever release and waive any and all rights, claims, and actions for damages that we, our heirs, executors, administrators, and assigns may have, or that may hereafter accrue against any and all persons, organizations, and other entities associated with the Move4Mona event, including, but not limited to Mona Foundation, and all of its officers, trustees, employees and agents, sponsors, affiliates, volunteers, and individual Move4Mona 5K Walk/Run organizers, arising out of or in connection with my voluntary involvement before, during, or after this event.

I verify that I am physically fit and sufficiently trained to participate in this event and I fully assume all risks involved in this activity. I further attest that I will be mindful of traffic along the racecourse, and hold said sponsors and organizers blameless in any harm that may happen.

I further consent to the collection and use of my Personal Information as contained in Mona Foundation Privacy Policy and give my full consent and permission for the free use of my name and/or pictures in related promotional activities including telecasts, broadcasts, newspapers, posters, advertising, and across Mona Foundation Social Media Platforms (Facebook, Twitter, Instagram, TikTok).
